This Adoption Agreement is made by BIG LOVE ANIMAL RESCUE which hereinafter shall be referred to as RESCUER. I agree to adopt this pet and by doing so, I accept the terms of this agreement set forth below. I understand and agree that the RESCUER, reserves the right to reclaim the dog if any of the conditions described below are not met. The RESCUER may also repossess the pet if there is reason to believe that the pet is being mistreated, abused [verbally or physically], neglected, put in unsafe or unhealthy conditions. I further understand that RESCUER representative may make a follow-up visit, at a mutually convenient time, to ascertain that all conditions continue to be satisfied. If I fail to comply with the terms of this agreement, I will reimburse RESCUER for any costs incurred by the organization to reclaim and remedy the breach of contract [lawsuits, etc.]. Please initial each of the following conditions below to signify that you have read and agreed to each.

- The pet will be housed at my home as a family pet, will sleep indoors at night, and shall have access to the residence. He/She will not be restricted to the yard or tied there. OUR POLICY IS TO PLACE ANIMALS AS FAMILY PETS ONLY. A lawsuit will be filed against any person adopting an animal from us for any other purpose[s] whatsoever, including Guard Dog service.
- I will provide veterinary care and treatment when necessary and proper nutritional food will be provided. Freshwater will be available to the pet at all times.
- I will socialize the animal with other animals for their well-being.
- A personalized identification tag with name and phone numbers MUST be kept on the dog at all times. The dog will be leashed on a martingale collar or harness and accompanied in public areas, streets, parks, etc. The pet will never be left tied up outside a store, coffee house while I am inside. The pet will never be left inside a car while I run errands.
- IF FOR ANY REASON I AM UNABLE TO KEEP OR PROVIDE ADEQUATE CARE FOR THIS PET, MY COMMITMENT WILL BE TO CALL BIG LOVE ANIMAL RESCUE TO SEE IF THEY CAN TAKE THE ANIMAL BACK. I WILL WORK WITH BIG LOVE ANIMAL RESCUE IN FINDING A NEW HOME. I will foster the animal for up to [3] months while working with BIG LOVE ANIMAL RESCUE on finding a new home. I will not sell or give this pet to any other person, friend, relative, organization, medical laboratory, rescue, pound, or animal shelter.
- If I move to a new location I will notify RESCUER of my new address and phone number. RESCUER MUST BE NOTIFIED IMMEDIATELY SHOULD THE PET BECOME LOST [also notify if deceased].
- I understand that a tax-deductible donation for cats is $300, adult dogs and puppies are $650.00.
- As stated on our website, the adoption fee includes a microchip, vaccinations to your pet’s adoption date, and spay/neuter at a veterinarian in our network. Once they are adopted, further medical including the next round(s) of shots and overall responsibility is then transferred to the new owner. It is important to keep a note of their original name as all their previous documents/records and future spay/neuter certificates will be logged in our files and at our vet offices under the original given name.
- I acknowledge and agree that males must be altered by six months of age and females by five months of age (A non-negotiable timeframe), and will contact the Rescuer to make arrangements at Rescuer’s veterinarian (Location- Los Angeles, Canoga Park, West LA, Sherman Oaks, Irvine). Spay/neuter will include anesthesia, procedure, pain injection, antibiotic injection, and a cone. Anything additional is at the expense of the owner.
- If Adopter chooses to use its own veterinarian for surgery, Adopter agrees to make Rescuer aware of such arrangements ahead of time. The rescuer will pay directly to Adopter’s veterinarian up to $125.00 a REIMBURSEMENT FOR THE COST OF THE SURGERY on the day of the procedure. The rescuer must receive the spay/neuter certificate directly from the Adopter’s veterinarian the same day the surgery is performed.
- Once you receive your pet’s microchip number, you must immediately log onto https://www.foundanimals.org/microchip-registry/ to register your pet as the microchip number is not yet registered to anyone and its purpose is void if the animal is lost and the owners cannot be contacted in such case. When registering, you will be given the option to list several contacts — please list Big Love Animal Rescue as your third contact in case of an emergency (our phone number is 310-403-2202)
- We require that each of our adopters commit to watching “Pet Fooled” on Netflix/YouTube. This documentary will forever change the way you feed your pet.
- From this day forward, I hereby release RESCUER from any liability for injury or property damage arising from my ownership of the pet. I have read and understood the terms of this agreement and I sign it freely and voluntarily.
